This is a fillable PDF form based on the Opening and Closing Credits scripts I use when I’m recording an audiobook that will go out on both ACX (Audible) and Findaway Voices (Spotify) and all the audiobook distributors that work with them.
It makes recording the credits easier whether you’re an author narrating your own book, or you’re narrating someone else’s book because:

Standard Audiobook Credits
These credits are standard copyrighted books, whether they are first editions, or have come out with later editions
Audiobook Credits with Updates
These credits are for copyrighted books whose authors/publishers have changed the content of the original book by less than 20%, which is the threshold for a new edition of the book, so they have the original copyright date, and an “Updated” date to include in the credits
